Thank you for considering our hospital as your pet’s provider of veterinary services. We are dedicated to maintaining the health of your pet and look forward to many future years together.

Cancellation Policy: If an appointment is not confirmed by 7 am the morning of the appointment, it will be cancelled. Clients will be offered the opportunity to reschedule their appointment.

Tardiness Policy: Clients who are late to their appointment may need to reschedule for a later date.

Difficult Pet Statement: Dunkirk Animal Hospital reserves the right to refuse service to pets that pose a risk to the safety of staff members and clients.

Please complete this form the same day as you schedule your appointment. This will help expedite the registration process and give us valuable insight in providing optimal care for your pet(s). The required sections have a red * asterisk. New clients need to arrive 15 minutes prior to their appointment to complete enrollment.
